If your child has any medical issues or allergies that we need to be informed about, please complete these forms and hand them in to the school office.
Additionally, if your child requires any medication to be administered in school please complete this form: Consent for the administration of medication
If your child is asthmatic and requires an inhaler, please provide us with a clearly labelled inhaler to be kept in the classroom. Please also complete this form: Consent for the administration of medication – Asthma
